Ujjain pairs most naturally with Omkareshwar, about 140 km away, which is the other Jyotirlinga in this stretch of Madhya Pradesh, set on an Om-shaped island in the Narmada. Doing Mahakaleshwar and Omkareshwar together is a well-worn two-Jyotirlinga circuit that many pilgrims complete in a couple of days, and it is the single strongest add-on to a Mahakal visit.

For those extending further, Trimbakeshwar near Nashik, roughly 400 km southwest, is another Jyotirlinga and a logical onward stop on a longer temple run toward Maharashtra. Devotees drawn to Shakti worship sometimes push east to Maihar, about 500 km away, where the hilltop shrine of Sharda Devi is reached by a long climb or a ropeway. Within Ujjain itself the town is dense with shrines tied to Mahakal, so most visitors combine the Jyotirlinga with the city's other sacred sites before travelling on.
